I'd have to check my records to confirm it, but I'm pretty sure that Swansea City left getting into the play-off places until the last day of the season when they went up from Division 4 in the 1987-88 season, when I was living there as a student.
Back in those day, the play-offs involved three sides hoping for promotion plus one trying to avoid relegation from the division above: Swansea beat third division Rotherham United 2-1 over two legs (meaning Rotherham were relegated), then beat Torquay United 6-5 over the two legged final to win promotion. What made their promotion even more remarkable was that at Christmas that season they'd been as low as fourth from bottom, and in the end-of-season run-in I saw them lose 2-4 at home to Crewe and 0-3 at home to Rochdale. However, good away form and a last day victory over Darlington* just scraped them into the play-offs for the first time that season.
*I wasn't at this match, but instead watch Portsmouth being relegated at Manchester United, but I can remember waiting outside WHSmiths at Piccadilly station for the Manchester pink'un to arrive so I could find out the Swans' score.
